Unfortunately it is not unusual that a custodian may amend (or revise) a Form 1042-S previously provided to the QI. The first thing to do is to make sure that the changes made by the custodian are correct. If the information on the amended Form 1042-S is correct then the QI most likely will also need to amend Form 1042 and Forms 1042-S.
